Choose Perioflow if you already own (or are buying) an EMS Airflow Prophylaxis Master or GBT Machine, want erythritol-based subgingival biofilm management, and value GBT protocol integration. The Perioflow handpiece is part of the EMS integrated unit — not a standalone purchase.

Choose Perio-Mate if you want a standalone subgingival handpiece that plugs into your existing high-speed coupling without committing to a full EMS unit. NSK Perio-Mate works independently and is comparable in clinical outcomes for routine subgingival biofilm management.

The clinical case for subgingival air polishing

Hand instrumentation alone cannot fully remove biofilm from rough subgingival surfaces — dentin, exposed cementum, implant threads. Subgingival air polishing with a low-abrasion powder (erythritol or glycine) reaches what hand instrumentation cannot, with significantly less tissue trauma. The European Federation of Periodontology supports air polishing with low-abrasion powders as part of stage II-III periodontitis management and peri-implant maintenance protocols.

For routine recall hygiene where pockets are shallow (<4 mm) and biofilm is supragingival, you don't need a dedicated subgingival handpiece. The decision becomes relevant when your case mix includes meaningful supportive periodontal therapy (SPT), peri-implant maintenance, or stage II-III periodontitis treatment.

Equipment context matters

If you're EMS-equipped

Perioflow is the natural choice. It's already integrated into your EMS unit — you don't buy a separate handpiece, you buy an EMS unit (Airflow PM or GBT Machine) that includes it. The clinical workflow is integrated with GBT and the erythritol powder is patent-protected, so the entire ecosystem is internally consistent.

If you're NSK-equipped

Perio-Mate is the natural choice — it's a standalone handpiece that plugs into your existing high-speed coupling and uses Perio Mate Powder. It pairs cleanly with the Prophy-Mate neo for supragingival work (Step 1 of MIT) and the Perio-Mate handles Step 2.

Perio-Mate is much lower capital cost than buying a full EMS unit, but you still need a working ultrasonic scaler in the operatory for the rest of the periodontal pathway.

If you're equipping a new operatory

The decision becomes harder. If you'll buy a full integrated unit anyway, the EMS Airflow PM / GBT Machine bundles Perioflow into the price — incremental cost is small. If you'll start with a scaler + add air polishing later, the NSK route (Varios scaler + Prophy-Mate neo + Perio-Mate later) is incremental and cheaper at each step.

Powder and consumables — the ongoing cost

EMS PLUS erythritol powder is substantially more expensive per kilo than NSK Perio Mate Powder (glycine). For a periodontal-heavy practice that goes through significant subgingival powder annually, the consumables gap matters. Multiply your annual subgingival use by the per-kilo price for each to get a realistic 5-year operating cost comparison.

Both use disposable single-use subgingival nozzles. The nozzle cost is broadly comparable across both ecosystems.
